Executive Summary
Our product and engineering team helped a digital therapeutics startup transform tinnitus care with a non-invasive, clinically grounded audio therapy platform. The goal was to deliver personalized 3D spatial sound tailored to each user’s tinnitus profile and evolving treatment plan, while giving clinicians the tools to monitor outcomes and adjust care in a compliant environment. We built a cross-platform mobile application for immersive therapy experiences, a tenant-aware web portal for clinics, and a secure backend that enforces HIPAA controls and aligns with FDA digital health guidelines. The result is a patient-centered therapeutic experience that pairs clinical integrity with modern audio engineering and operational reliability.
Problem
Tinnitus is highly individualized, and one-size-fits-all audio libraries rarely provide lasting relief. To be useful in practice, therapy needs to personalize soundscapes to each patient’s profile and adapt as symptoms and responses change over time. Achieving this at scale requires precise audio rendering, headset-aware playback, and session logic that can refine itself with patient-reported outcomes. At the same time, clinicians need a clear view of progress and the ability to tune therapies across cohorts, which calls for robust multi-tenant web tooling. All of this must be delivered under strict healthcare requirements: protected health information must remain private, access must be controlled, every action auditable, and the software lifecycle should reflect FDA expectations for therapeutic applications.
Solution
We engineered an end-to-end digital therapeutic that integrates mobile therapy delivery, clinical intelligence, and secure operations. The mobile application renders immersive, headset-aware 3D soundscapes and interactive therapy experiences that engage patients and reduce friction in daily use. Each session is generated by a proprietary algorithm that incorporates the patient’s tinnitus profile and ongoing feedback, ensuring therapy evolves with the user rather than remaining static. Session adherence, symptom updates, and outcomes flow back to the platform to inform subsequent personalization.
Clinical needs are supported through a dedicated web application designed for tenant-based clinics. Administrators manage onboarding and permissions, while clinicians review progress dashboards, inspect therapy histories, and adjust plans with confidence. Role-based access is enforced via Keycloak, separating responsibilities and limiting exposure of sensitive data. The platform maintains a comprehensive audit trail, giving teams the operational transparency required for regulated environments.
Security and compliance are built into the foundation. Services are containerized and orchestrated with Docker Swarm for fault tolerance and isolation. All sensitive data is encrypted in transit with TLS and at rest with AES-256. Two-factor authentication is enabled for administrative and clinical roles, and granular access policies ensure least-privilege across the stack. Development and release processes follow HIPAA safeguards and align with FDA digital health guidance, with logging, monitoring, and testing practices that make the system observable and resilient.
Delivery followed scoped, iterative sprints. We prototyped key audio experiences early to validate headset handling and spatial rendering, then layered in profile-adaptive session logic and clinician workflows. Regular reviews with clinical stakeholders kept the product faithful to therapeutic intent while we matured performance, reliability, and security for production use.
